Cannabinoids Reviewed
Cannabinoids affect sleep through multiple mechanisms. THC reduces sleep onset latency and increases slow-wave sleep but suppresses REM. CBD at 160mg improves sleep quality without REM suppression. CBN — in the first RCT (2025, n=189) — reduced sleep onset by 18 minutes and wake-after-sleep-onset by 22 minutes without next-day sedation.
THC activates CB1 receptors in sleep-regulating brain regions including the basal forebrain and hypothalamus, reducing sleep onset latency and increasing total sleep time. CB1 activation also increases slow-wave (deep) sleep, which is restorative.
THC suppresses REM sleep — the stage associated with dreaming and memory consolidation. Short-term REM suppression may benefit PTSD patients with nightmares, but chronic REM suppression impairs memory consolidation and emotional processing. REM rebound occurs upon THC discontinuation.
CBD's anxiolytic effects indirectly improve sleep by reducing the hyperarousal and racing thoughts that prevent sleep onset. CBD at 160mg/day has been shown to improve sleep quality in insomnia patients, likely through this mechanism.
CBN's hypnotic mechanism is not fully characterized but appears to involve GABA-A receptor modulation and weak CB1 agonism. The 2025 RCT demonstrated significant improvements in both sleep onset and sleep maintenance without next-day cognitive impairment — distinguishing it from benzodiazepines and Z-drugs.
First RCT (2025, n=189): CBN 40mg reduced sleep onset by 18 min and WASO by 22 min. No next-day sedation. Preferred for sleep maintenance insomnia.
CBD 160mg improves sleep quality. Most effective when insomnia is driven by anxiety. Does not suppress REM sleep.
